Who is Sunflower Support Coordination?
At Sunflower Support Coordination, we are dedicated to making a meaningful difference in the lives of individuals with disabilities and their families. Based in the heart of Toowoomba, we provide personalised, person-centred support coordination that empowers participants to pursue their goals and live the life they choose.
Just like a sunflower turns towards the sun to grow and thrive, our approach is centred on supporting each participant to move toward the opportunities that help them flourish. Our mission is to make each participantโs life easier, fairer, and more connected by maximising independence and encouraging active participation within their community.
We support participants to navigate the National Disability Insurance Scheme (NDIS), connect with the right services, and build the confidence needed to achieve their goals. Through an individualised, strengths-based approach, we recognise the unique abilities, needs, and aspirations of every person we support. By working collaboratively with participants, families, and service providers, we aim to create meaningful opportunities and lasting outcomes.
