Our mission is to support people living with disabilities to live the life they choose. As a local provider in the Darling Downs, we offer dedicated support coordination to help you navigate the NDIS, connect with the right services, and build the confidence and independence needed to achieve your goals.
What is Support Coordination
Support Coordination is a service funded through the National Disability Insurance Scheme (NDIS) that helps participants understand and implement their NDIS plan.
A Support Coordinator works alongside you to help you connect with the right services, understand how your funding can be used, and build your confidence to manage your supports over time. The goal of Support Coordination is to help you develop the skills and knowledge needed to get the most from your NDIS plan.
Think of a Support Coordinator as someone who helps guide you through the NDIS. They can help you find providers that suit your needs, organise your supports, and make sure the services you use are working well together to support your goals.
At Sunflower Support Coordination, we work with you to make the NDIS easier to understand and less overwhelming. Our team supports you to explore your options, connect with the right providers, and build your independence so you can get the most value from your NDIS plan and focus on what matters most to you.
